Just a reminder that all incoming kindergarteners and second graders need to have a completed dental form on file and all kindergarters and fifth graders must have a completed physical exam on file.

Children Who Receive Medication At School
All medication, even over-the-counter, needs to have a physcician order to be given at school. I have special medication administration forms that must be completed by the doctor and the parent every year.

Health Examination Requirements
The State of Illinois requires that all students entering kindergarten and fifth grade have a physical exam dated on or after 9/01/05 and all their immunizations must be up to date. In addition, all students entering kindergarten and second grade are required to have a completed dental exam on file by May 2007. You may give me the required forms whenever they are completed.

The State of Illinois requires that all transfer students from out of state present proof of a current physical exam completed on a State of Illinois Child Health Examination Form within one year before entering school. This form is available from your health provider's office, or from the school nurse.

Medications may be administered in school by the nurse when a Medication Administration form is completed by the child's parents and physician. This form must be completed for all prescription and over the counter medications. The form is available from the Health Services link on the bottom of this page. Click on the medication link to get the form. You may also get a form from from the school nurse.
